For years now, women have been subjected to wearing shoes that aren’t necessarily the best for their feet. History has shown with customs like chinese foot binding, that the well being of a woman’s foot has taken second priority to social norms and beauty. When I first started practice, I dealt with this contradiction in philosophy. As a foot surgeon, correcting deformities that cause pain was the basis of my training. But as time went on, I realized that advising women not to wear high heeled shoes, was unrealistic. This is because in the modern world, professional or formal attire requires a woman to wear high heeled shoes due to social norms and pressures. This is unfortunate but true.
Rather than isolate my practice form the realities of what women endure in the professional world, I decided to create a procedure that addresses the problems that women face when wearing high heeled shoes. One such problem is a wide foot. Bunions and Tailor’s Bunions (smaller bunion on the outside of the foot) are often the cause of pain and constant grief for women that have to wear these kinds of shoes.
The Cinderella Procedure addresses this issue with a modern, pain free, and short recovery procedure. Like my Aesthetic Bunion Correction, the Cinderella Procedure allows most patients to walk right after the procedure and be ambulatory during the post-operative period. In most cases, patients are able to be back into high heels in 4 weeks. This is a remarkable improvement form the traditional method of correcting this problem that leaves patients non-weightbearing for up to 2 months in some instances!

I am a 49-year-old female. I have bunions on both feet since childhood; I believe it’s genetic (my dad had bunions). I have always considered surgery to correct them because I hate the way they look, not because of pain or discomfort. I’ve been told that surgery can sometimes cause pain which is definitely unacceptable for me. Anyway, I saw the Cinderella procedure on The Doctors and thought this might be something that I would really consider. Is this procedure covered by health insurance? If not, what is the cost?

It saddens me to have to inform patients that the procedure that I do is not being done else where, being that I created this technique of bunion correction. Although bunionectomy and tailor’s bunionectomy are procedures that other foot surgeons perform, the unique approach that I have developed is not. The doctor’s at Scripps are some of the best in the country, and I have a high level of respect for them, however, like most foot and ankle surgeons, they are still performing procedures that were developed decades ago.

Thank you for your question… I’m impressed! Proximal Phalanx… are you involved in Podiatry or have you done your research? Either way, excellent observation! Yes, this procedure that I created (Aesthetic Toe Shortening) is my favorite. The extra skin does not cause problems, in fact, I want a little extra skin there for the finished aesthetic result. The procedure is significant being that there is NO evidence of surgery when we are finished… this is currently not available anywhere in foot and ankle surgery. The procedure is completely innovative. What I strived to achieve when I created this procedure 5 years ago, was to provide women a simple, pain free, short recovery solution to hammertoes or long toes with a completely aesthetic invisible result. 5 years into it, and thousands of toes later… I can proudly say that we did it!

3/19/2009 2:26 I had an extremely painful surgical correction of my right lateral bunion and synovectomy of my 2nd toe ~20 years ago by “the best orthopedist/MD) in Phila. Within a few years, the screw in my foot began to “wiggle” out and finally I demanded that the surgeon’s partner remove it -despite his objections – which relieved the pain caused by the screw and have had no further problems except that the MD “forgot” to tell me -before surgery -that my 2nd toe would not be as long as my 3rd toe. This was VERY frustrating since I was trying to achieve an aesthetically pleasing look along with the relief of foot pain I was experiencing. In any event, now my left foot needs the same – or similar – work. Since it appears that you are the only one doing these procedures, I must ask, does the patient feel the screw in the 2nd toe? That is, are the 1st and 2nd or 2nd and third phalanges of the 2nd toe screwed together and does this prevent flexibility in that area. When I asked my orthopedic surgeon(pre-op), his cavalier response was that I ‘wouldn’t be able to pick up a pencil between my fist and second toes”. With that remark as background I am impressed with your sensitivity to the plight of women who can not wear fashionable shoes any longer. It is tremendously demoralizing. Shoes complete the look of any fashion-conscious woman. The only thing I can think of as a way to make a man understand is to ask him how he would be able to function in the business world if he were unable to wear a tie? In any event, the point is, does your surgery prevent the patient from ever wearing flip-flops or thong shoes post-op? This is the question I should have asked my prior surgeon. Also, after my foot surgery, the calf on that leg began to develop a loss of muscle tone. No orthopedist has been able to explain it, but my thought is that by the transplantation of the ligaments and tendons that attached to the phalange that was removed were also removed and it somehow hindered the nervous or muscular innervation of that particular aspect of the calf. Can you comment?explain? Also, since you have only been doing this procedure for ~ 5 years(?) do you have any idea as to the long term effects of this procedure? I realize that being so new a procedure this is hard for you to forecast. I am a 62 year old female,retired, (I look ~40-at least that’s what people tell me -and without any plastic surgery or botox) and without any other major problems except bilateral hip replacement done by Richard Rothman(a hip implant pioneer) with EXCELLENT results(~10 years ago). This certainly sounds like the procedure that I need. I am 52 and have bunions on both feet, worse on my left. I can no longer wear beautiful shoes and I don’t want to be told again to wear a wider size,unattractive shoe and live with it. I do have pain and that is a concern too.

Hello, I as many others with horrid bunions watched with amazement your procedure on the Doctors. The bunion I have is getting to the point that regular shoes (not heels) are difficult to wear. My Mother also had a bunion on the same foot. I have never even attempted any type of conventional surgery due to the very low success rate I have read an heard about, I have never heard anything good about conventional bunion surgery. I cannot afford to be crippled for months and then not know if your foot will be bunionless without wires, screws of all types, etc. May I ask how long you have been doing this procedure and how would flying affect the surgery site afterwards, I live in Illinois. I also have horrible toenails which have been in this condition for several years after a particular virulent bout with Athlete’s Foot. It seems that the skin under the nails is growing upward, pushing the nail up, of course I never, ever wear sandals or wear a shoe that is open toed. I have had bunions since the age of seven so obviously mine are genetic and I never could wear heels. I’ve been wearing podiatrist made orthotics since 1990 and still my bunions continue to worsen. Now I am developing severe tailor’s bunions which are causing more pain that the original bunion and it’s becoming increasingly difficult to walk. I can barely fit into any shoes as well.
I had a traditional bunionectomy in my right foot in 1990 and the surgeon botched my foot quite frankly and I wound up with my bunion back – while my foot was still in the cast – and was left with a huge, painful scar and no longer have full mobility of my first metatarsal.
I do not want to go through that again but obviously something needs to be done. Does your Cinderella procedure work on severe bunions (e.g. angles over 22 degrees) and is there an age that is considered to be too risky due to arthritis?
Will you be teaching other podiatrists your procedure in the future so women (and men) around the world can benefit? I live in Houston and have family obligations; would I be able to fly back in a few days? Would I be able to walk up stairs? Is this procedure covered by insurance if it is medically necessary? What is the cost of the procedure for both the regular and tailor’s bunion?

I just had this procedure done on both feet- not by Dr. Sadrieh, I am in the Midwest. So far, I am very impressed. My doctor performed the exact type of surgery shown on “The Doctors.” I had the second foot done yesterday and can walk around for short periods. My first foot was done about 3 weeks ago and though it is a bit swollen, it looks beautiful. From the research I did, doing it sooner rather than later increases your chances of success. My insurance is covering it totally and I had no idea that this procedure was anything close to “cosmetic surgery” until I happened to catch that episode of The Doctors. If you have bunions, I think it’s as much of a medical issue as cosmetic- if you approach it with your doctor as such, I can’t imagine insurance companies viewing it as plastic or elective surgery. Hint- my doctor said that when women come in and complain only of aesthetics, he doesn’t take them as seriously.

Wow! Gotta love such an innovative Doctor!! I have the same questions that Julie from March 19th had. Had xrays done of both feet last year with plans to have the surgery done this summer. My othopedic surgeon still is using the old method with the K wire which would leave me off my feet for weeks and using crutches for more than a month. My concern is that I have had a DVT in my right leg which would reguire me to be on Lovanox injections while being off my feet and the risk of another DVT frightens me. With your procedure this would get me up and moving with a faster recovery and limited risk. Both feet have both bunions as well as some hammertoes. YUCK!!! I am 49yrs old healthy and active. With the old procedure they would do on the left foot, both bunions and 2 hammertoes. Can I have this much done and still walk out of your office? I would love to have my films sent to you and talk about setting up surgery. We have a friend who lives in the area for me and my husband to stay with. I’m from Oklahoma. Not until I was 11, did I notice anything was wrong with my feet. Both of my second toes on my feet are half a centimeter or more longer than my big toes. They are also noticeably wider than my smaller toes. As if that weren’t enough of a problem, I have always had problems with them when I would run. They would hurt like crazy! I also have a question about nail regrowth. I’ve had ingrown surgery and my big toes look very narrow. Is there anything I can do this? I’m pretty unhappy with the appearance of my feet entirely.
I am interested in getting this procedure done even if it requires me to travel to you. This has been a problem for me for way too many years and I’m so excited that there is now some help. I need three toes done on both feet. I understand that you do one foot at a time. Would I be required to stay nearby for recovery just in case?? I live across from Detroit in the suburbs outside of Windsor, Ontario. The show “The Doctor’s” on television was timely as I was just at my doctor’s office this week and discussed my bunions. She is a well-informed doctor and informed me that the surgery would be painful and to post-pone it as long as possible. My mother has and still has bunions, even after surgery, and I am genetically predisposed to this ailment.

I’m in pain. I had a shot on my foot to stop the pain, but a month after the pain started again. I cannot wear hardly any shoe even after inserting the caushion the doctor recommended.
I saw the cinderella procedure on TV two weeks ago. I though a angel let me see it while I was at the fitness center. I’ve talked around to other people and they are impressed about it as I am.

Can you combine Cinderella procedure with the liposuction of the ball of the foot? The narrowing the way you do it a great idea – you literally push the first and fifth mettarsal bones inwards and fix them in the new position. But in some people like myself there may be another obstacle to wearing narrow shoes – the fat tissue on the foot, normally concentrated in the ball of the foot, is over developed. This acts like cushioning but when there is too much of it, it makes it difficult for the foot to be squeezed into the shoe. Nothing to do with general obesity ( I am BMI 23) and you cannot influence it by dieting. I feel that the effect of Cinderella procedure would be magnified by combining it with the lipo of this area ( ball of the foot). Is this something you do or would consider doing or would I need to find a different surgeon to perform lipopsuction and can you help me with that by making some suggestions? Obvoiously I would prefer if you were able to do the lot. I will definitely phone Shu Shu do discuss my case – I was just hoping for some general guidance and there may be other people with the same problem wanting to know about this.

Hello Daisy,
Thank you for visiting and commenting. Corns are a result of a hammertoe toe or a contracture of the toe that results in rubbing of the skin and thickening over time. The reason for the toe to contact can be one of many. The toe can be too long, or the mechanics of the toe can be faulty creating a hammertoe. Ultimately, they all require what is called hammertoe correction.
Traditional hammertoe surgery requires an incision on top of the toe that will result in an ugly scar. The procedure also requires the use of a pin or K-Wire which will stick out of the toe for 4-6 weeks. These all results in a less than aesthetically pleasing result.
The procedure that I have created (The Perfect 10!) allows you to correct the problem without any evidence of surgery. The incision is on the side of the toe and there is no pin sticking out of your toe. This reduces the recovery time from 4-6 weeks to only 2 weeks.
The callus or corn will slowly disappear since we have corrected the toe structure. This is another mistake that I see often where doctors will remove the corn by cutting it out, leaving the patient with a scar instead of a corn.
I recommend you consider a consultation where we can review x-rays of your feet and give you a specific solution for your condition. You can ask for Shu Shu (my office administrator) and she can co-ordinate the process for you.

Hello Dr. Sadrieh. I’m glad to know you have answered questions from us! Well, I’m 24 years old and very fashion driven. I have a business so I’m always on my feet, high heel feet! I have bunions on both feet but one stands out more than the other. I have been wanting to do this since I was little (being teased by 2 older brothers) I do have pretty toes but the bunions hurt after a while being in certain shoes/heels, resulting in corns and blisters and scars! I always do research on what I can do to stop this but with the K wire method, I just couldn’t bare with the long dowtime and the risk of it coming back years later. I even bought the toe separater from Walgreens to relieve pain after all days work.
Anyway, thanks to The Doctors show, I saw you and this procedure done to a female patient! I read you are the only one that performs this.

We see a lot of out of state patients! I will have Shu Shu (our office administrator) contact you to guide you through the process. We have a car that will pick you up and take you to your hotel, and take you to and from your procedure when the time comes. I actually have a high % of my patients from New Orleans!
2) Is there a slight chance of bunions returning years later?
Once I see your xrays I will give you a more clear picture of exactly which procedure I will be doing for you. But regardless of this, 95% of the bunions I see and correct are done in a permanent manner. The procedure will actually change the position of the bone so that the underlying deformity is corrected.
3) How much would the procedure cost?
The procedure is usually covered by insurance. If you do not have insurance, Shu Shu will provide you with a breakdown of the costs for the procedure.
4) Can I still wear my heels?
Yes! Isn’t that why we’re fixing them
High heeled shoes will exasperate the problem if you have an underlying genetic predisposition. This means that if you were born with the genetic potential to develop bunions, they will advance and progress if you were certain kinds of shoes or certain activities. Once we have corrected the bony deformity, the shoes will not contribute to recurrence.

I had a bunion removed 5 years ago. I also have a screw in my foot. In the last week, my foot has been very painful. Originally it woke me up in the night. I thought the pain would eventually subside but it has not. It is not constant but it does come and go and now I am feeling more pain up and down my leg. It appears that it extends further and further up as the days go by. Could this be some kind of infection in there or is it maybe just due to the cold weather. I thought it might be the weather but then it hasn’t happened in the past 5 years and today was rather warm. Still hurting. I am also very dizzy today and now my arm on that side is hurting. Could be due to something totally different but thought I would mention it just in case they could be linked. Can you give me any more advise or information on what it might be that is causing so much pain?

I am very interested in the cinderella procedure or the tightrope. I have not found anyone in Minnesota that is able to do it. I am 37 years old and have had bunions ever since I remember. I just want normal feet. Help! Is there anyone that does this in MN or what would the process be to come there. I have Health partners Insurance and also wondering if this is something that is covered. I even try to were the bunion regulators at night with no luck.
hello, first off, wow. It is a beautiful thing that you do helping these women. Um, okay, I believe I need nail reconstruction and aesthetic toe shortening. My feet have been like this since I was 12. Taking care of my brother for nine years in Boston until he passed from leukemia, forced me to put my needs aside. Then my full time job as an LNA in a geri-phych unit from age 16 to present, has also contributed significantly as to why I have had to let my painful nails get so bad. Im 25 now, and I am ready to do something for myself. I have never worn open toe or peep toe heels or sandels, and with my group of almost perfect looking close girlfriends, its been hard. Boy, sorry I sound vain. They have been wonderful friends for 15 years and encourage me to do this. It hurts to even wear regular shoes. My big toes and even the sides of my feet hurt terribly, and I hope I havent waited so long that the nail reconstrution on my big toes wont be possible (I will email you pictures). I used to be a runner, and now I cant even do that. People say Im pretty, but they wouldnt if they saw my feet, I wear socks 24/7.
Anywho, I have good credit and everything, but I already own my own home and barely supporting myself. I could never afford to fly from NH to Beverly Hills. I was hoping you could please email me a name of a surgin you trust that works in the Boston area? It would mean the world to me. Thank you for taking the time to read this…
what does a procedure like this cost?
A funny thing happened on the way to have my foot surgery. I was set up to do the traditional bunion/joint surgery on my big toe. I was sent to my D.O. to do the pre-op, which included an EKG.
Well my EKG was abnormal. I had never had an abnormal EKG before. So I went to my husband’s Cardiologist at his nudging. (I was going to ignore it and figure it was just a fluke). Well boy am I glad I didn’t. During my cardiac cath my aorta collapesed and I had to have emergency heart surgery with two bypasses.
So now (two months later) I am back on my foot. So I am looking for someone in Texas that does the “tightrope procedure”. I really do not want to have to have a 6 week recovery and have to stay off my feet.
So is there any on who you can suggest that can help me out? I would really appreciate it.

I saw you on The Doctor’s TV way back in June 2009. I am flat footed, and last Friday was told that “If you stay active, you will need a new knee.” Of course I plan on staying active – I walk when I play golf, but my feet do suffer even though I wear orthodics. Would you please tell me if your surgery to correct flat feet is covered by insurance and are you the only doctor that performs this surgery? Also what is this surgery called?
